Helix DNA Test Specific - Protection of Privacy Declaration

 At Skin Rejuvenation Technologies we are dedicated to protecting all information that is collected by us which includes;

1. information that you share with us through user consent or registration
2. genetic information obtained through DNA testing services that we offer.

 We are compliant of all existing laws and regulations in this regard.

 1. Information that you share with us through user consent or registration

Information that we generally collect will comprise of the information that is directly shared with us through completing the user consent form, and registering this information on our online booking system. We process this information for the purpose of providing our services. Your information will only be shared with those at Skin Rejuvenation Technologies who are conducting the genetic screening and compiling your personal results. The information that you provide to us will not be shared with anyone outside of Skin Rejuvenation Technologies, unless required by a court order.

2. Genetic information obtained through DNA testing services that we offer

With your consent, we extract your DNA from the provided buccal swab and analyse it according to the genetic test panels you have selected, to provide you with your unique DNA reports. Your DNA sample will only be used for the genetic test panels that you have selected. You may request that your DNA sample be discarded after the tests have been completed.

 Please note that we at Skin Rejuvenation Technologies are dedicated to protecting the privacy of children, and therefore anyone under the age of 18 will be under the responsibility of the parent or guardian. The parent or guardian will need to perform the buccal swab and complete the user consent on behalf of the child. 

 Personal information and genetic information will not be directly associated with each other. All information is stored in a secured and encrypted database which only authorised employees have access to. Technical and organisational measures have been initiated to protect the integrity and security of all information. This information will not be released to any third parties, or distributed within Skin Rejuvenation Technologies unless required by law.

Skin Rejuvenation Technologies will not contact you when new genetic information becomes available on the variants you have been tested for, nor will they contact you regarding any new genetic test offerings that become available.

Disclaimers

The reports generated by Skin Rejuvenation Technologies as well as other verbal consultations with employees at Skin Rejuvenation Technologies do not serve as medical advice. The results and recommended guidelines laid out in these reports are intended to highlight potential health risks and promote healthier lifestyle choices. Should you require specific medical advice, Skin Rejuvenation Technologies suggests consulting with your health care practitioner.

 Any results obtained in your genetic reports does not replace medical advice, diagnosis or status given to you by your health care practitioner. The results represent genetic predisposition as understood at the time of testing, rather than a medical diagnosis and should be interpreted as such. Should you have very specific concerns regarding your genetic results you can consult with your health care practitioner and/or genetic counsellor.

 Limitation of Liability

Skin Rejuvenation Technologies tests and reports are neither comprehensive nor absolute. Scientific advances may deem the current tests and reports, inaccurate. If that is the case, the information supplied may not apply to your case due to the information being out-of-date. The tests are reliant on the scientific advances at the time of testing. We will provide follow up feedback, or updates if you require a new and separate testing done or you request a separate follow-up due to new developments.

Your report constitutes the full extent of information to be provided to you under this agreement.

Skin Rejuvenation Technologies makes no warranties or representations, either express or implied, that this service will meet your requirements or expectations. No advice or information, whether oral or written, obtained through or from the provision of these services, will itself create any warranty or representation or other obligation.
